I, the parent or guardian, consent to my child or ward participating in the playing of hockey under the Canadian Hockey Association (CHA) player registration certificate. I agree that my child or ward does become subject to The rules and regulations and decisions of the CHA, its Board of Directors, its Branches and/or divisions, which may be Restrictive in some areas such as movement from team to team, conduct, etc., and I hereby consent to him/her Participating in the playing of hockey with the said CHA, its Board of Directors, its branches and/or divisions. I am aware That these rules and regulations are available to me through my child's or ward's team manager. I also agree to abide by the GHA's Fair Play code of conduct as parent/guardian of above noted child(ren).
